Bullhorn lift – small procedure, big effect

As we get older, the distance between the nose and the upper lip increases. If this distance is, for example, 1.5 cm at the age of 20, it can increase to 3 cm by the age of 40, or even 4-5 cm at a higher age. A bigger distance between the nose and the upper lip is therefore a typical sign of ageing and is usually considered unattractive. Throughout our lifetime, the lips also lose their volume, which heightens the effect of drooping, thin lips.

In addition to surgical methods to lift the face and rejuvenate the skin, another procedure in plastic and aesthetic surgery has proven effective – the so-called bullhorn excision.

What is a bullhorn lift?

The term bullhorn lift, or bullhorn excision, refers to the rejuvenation of the face by performing a simple outpatient procedure under local anaesthetic to remove skin between the nose and upper lip. This makes the face appear younger and lifts the upper lip, and as a result it is sometimes referred to as a lip lift.

While the average age of the patients who undergo this procedure is between 50 and 60 years of age, overall patient ages range from 30 to 80. This also depends on the patient’s genetic predisposition.

How the lifting procedure is performed at the Klinik am Rhein

Bullhorn method

The procedure is performed under local anaesthetic as an outpatient surgery and takes around 30 minutes. After injecting the anaesthetic, the surface of the marked area of skin is removed. This is a narrow horizontal strip of skin and tissue underneath the nose. Here it is ensured that the natural lines in the skin are followed. The name bullhorn excision comes from the lines drawn on the skin that resemble bull horns.

The wound is finally sutured together again in several layers to complete the surgery. Around 80% of the fine, almost invisible lines are concealed in the natural fold of the lateral nasal wing and in the front part of the nostrils (see sketch).

After-treatment of the bullhorn method

Patients can leave our clinic on the same day.
The fine sutures are removed after a week and the patient can resume all work and social activities. There is almost no swelling and pain, and can be improved by cooling.

Nice side effect – lip augmentation

The main goal of this small but extremely efficient procedure is to make the face appear younger by shortening the skin on the upper lip – a nice side effect is that it also emphasises the upper lip by lifting it slightly.

Lifting the lip not only makes it look fuller and more visible, but appears very natural. Therefore, there is no need to inject fillers such as hyaluronic acid into drooping lips to enhance and plump them up. The bullhorn surgery aims to improve, harmonise and rejuvenate the appearance in a natural way. Many patients say that they have a bigger, more open smile after the surgery. As a result, the treatment is also known as a ‘bullhorn lip lift’.
